Before Your Appointment
Please verify your benefits and confirm that Kleiman Healthcare Services is in your plan’s network. Your insurance card and a photo ID are all you need to bring. When you book online you can select your insurance plan, and our team can help with any questions.
Deductibles & Copays
A copay is the fixed amount your plan asks you to pay at each visit. A deductible is the amount you pay for covered care each year before your plan starts paying its share. Depending on your plan, you may owe a copay, part of the visit toward your deductible, or nothing at all — your plan’s summary of benefits spells it out.

Not using insurance? No problem. We publish transparent self-pay pricing for primary care and women’s health visits, plus Essential Care and Weight Loss membership programs.

What if I don’t have insurance?
No problem. We offer transparent self-pay visit pricing and membership programs, and qualifying services may be financed through our financing partner, Cherry.
